Please limit your essay to one (1), typed page. Please submit a letter of recommendation from your Local usher president, State usher president, and your Pastor. If you hold either of these offices, your vice president(s) must prepare a letter to be submitted with your application. NO APPLICATION WILL BE PROCESSEDWITHOUT THE REQUESTED INFORMATION. *** PLEASE NOTE***Each applicant’s state must submit a minimum of $350.00 in T‐Shirt sales to be eligible for consideration of a scholarship. If more than one applicant applies the total sales for that state must increase by $350.00 per applicant.
